Freelance Brief: Creation of Google Forms for Clinic Use

I’m currently looking for someone to support me with a project that involves rebuilding a set of clinic forms using Google Forms. Previously, we used a clinic management tool with built-in consent forms, but much of that setup is now outdated and no longer fit for purpose. The key issue is that the old forms didn’t allow us to extract usable data or generate reports from the responses — something we now need to do moving forward.
I’ve gathered a fair amount of content and structure for what these forms should contain, but I’m struggling to find the time to complete them myself. I’m hoping to find someone who can:
Take the information I provide and professionally structure it into a series of clean, user-friendly Google Forms.
Apply good form logic where needed (e.g., section jumps, conditional fields).
Ensure all forms are set up with timestamps, required fields where appropriate, and formatted clearly for both client use and internal record-keeping.
Deliver the completed forms with full access and ownership transferred to our Google account so we can manage and store them securely going forward.
There will likely be multiple forms required (client consent, media consent, treatment session notes, aftercare confirmations, etc.), so this could become an ongoing project if the first phase goes well.
