**Project Details:**
- Convert an existing paper form into a digital format - Two potential options: 1. Create an online form that can be filled out and stored on the client's website 2. Develop a fillable PDF form that can be completed, saved, and printed
**Requirements:**
- Proficiency in web development technologies (HTML, CSS, JavaScript) for the online form option - Experience with PDF form creation tools for the fillable PDF option - Ability to ensure data security and proper storage of submitted information - Strong attention to detail to accurately replicate all fields from the paper form - Excellent communication skills to discuss requirements and provide updates
**Deliverables:**
- Functional online form integrated with the client's website OR a fillable PDF form - Basic user guide for form completion and submission - Any necessary backend setup for data storage (if applicable)
