We are looking for a simple but professional two page document.
The document will be a template allowing certain field to be filled in. Mostly looking for the design to look clean and professional so there will be no images or graphics just layout, colour and spacing.
One page - Will need space to add image top right and image below it, title on the left and below this will just be fields to complete.
Second page - Will be fields to complete, this is a bit of a review page after reviewing a consumer and asking questions.
